RI hospitals adopt process to limit surgical error

Anesthesia error results in a surgical near miss
6/30/09--A surgeon anesthetized the incorrect eye of a patient about to undergo eye surgery at the Miriam Hospital this month, but the error was discovered before the patient entered the operating room.

Local Hospitals Trying to Reduce Medical Errors
6/29/09--In 2008, 89 sentinel events were recorded, and the much higher number is attributed to the better, more thorough reporting that began in 2007. The Utah Department of Health is now working to streamline procedures to reduce human errors in local operating rooms.
